Given by Prophet Luna in Vanaheim.

Prophet Luna has shown you a glimpse into the future...
Grants random Heroic.
Now the question is when?

After killing the last monster, it will spawn a portal on the monsters position which grants the player access to Possessed Luna.
Killing Possessed Luna grants you the foreseen Heroic item and a random Possessed Luna boss drop.
